- Up to 2 Fluoride Treatments When Indicated (ages 13 & younger)
- $200 OFF Sleep Apnea Appliance

Program Exclusions & Limitations
This is a savings plan, not dental insurance. It cannot be combined with any other insurance. It is only valid at this dental office; care from other providers and specialists is not included. Plan fees are subject to change. For complete details, see Plan Agreement or Plan Terms and Conditions.

** If periodontal infection is present, a periodontal plan may be required at an additional charge.

Dental insurance is too valuable to waste, and at Hite Family Dentistry, we do everything to ensure you get the maximum benefit you are entitled to under your plan. Our knowledgeable team members are well-versed in working with insurance and provide pre-treatment estimates as accurately as possible, understanding that the final benefit determination is with your insurance provider. Regardless of who your insurance is with, we will file and process all claims on your behalf.

Payment Options
We are pleased to accept cash, checks, and credit and debit cards, including Visa, MasterCard, and Discover. If you would like to finance all or part of your dental costs, we also accept Cherry, Proceed, Sunbit, and CareCredit third-party financing. In-house payment plans are also available.
